DUTIES
AND
RESPONSIBILITIES:
Complete training modules as required at www.infantva.org. Participate in
Kaleidoscope
training as required. Identify Part C eligible children. Develop and
monitor
the Individual Family Service Plan (IFSP). Participate as a
multidisciplinary
team member. Make teaching contacts as indicated in the IFSP.
Participate in
transition planning. Ensure the family’s procedural safeguards. Assure
confidentiality of children, families, and co-workers. Maintain a safe,
healthy, clean and orderly work environment. Performs other duties as
assigned.
QUALIFICATIONS
REQUIRED: Minimum
of a Baccalaureate degree in a human service, educational, or health
related
field. Must be able to lift a minimum of 50 pounds. Must be familiar
with all
backgrounds of families in the program. Must demonstrate maturity and a
high
level of energy.
QUALIFICATIONS
PREFERRED:
Masters degree in a human service, educational, or health
related field.
